Be A Volunteer Driver
Since Community Transit cannot accommodate every request using buses, volunteer drivers are a critical link that allows people in our rural areas to access the medical services and social activities necessary to maintain a high quality of life. Community Transit provides volunteer driver service to Southwest Minnesota. Let us know you’d like to volunteer.

Volunteer DRIVER Forms
If you are interested in becoming a first time Volunteer Driver, please complete a New Volunteer Driver Packet. The packet consists of:
- Registration Form
- Code of Conduct
- Informed Consent Form
- Medical Statement
- Vehicle Inspection Report
- Reference Check Form
- Privacy Notice
- Background Check
If you are already a Volunteer Driver, click the links below to print off renewal paperwork any time throughout the year. You don’t have to wait until you get a letter from UCAP. Have the required forms on hand for your next appointment!
